Is the Maladaptive Daydreaming scale test public?

I'm writing an essay on maladaptive daydreaming for English and in trying to do research for it I found a lot of information on the maladaptive daydreaming scale by Eli Somer but I haven't been able to find the actual test anywhere. I've found some sources which shared sample questions but never actually found the whole thing.

I was wondering if anyone knew if the full thing is actually released to the public and if it is where on earth I could find it because I'm extremely curious now just because I haven't been able to.
